ELX550AT2 PHY failed to be recognized on the PCIe Gen2&3

x550 functionality on PCIe :

This device is integrated into my own designed board. The board design has followed Intel design recommendations. 

Intel device is not recognized on the PCIE , on board see clocks & PCI signals for a moment and then silent

PCIe interface was checked with another non Intel product without any issue. what can be possible reasons ? What debug option are available ? Any known errata relating to this phenomena ?

In order to better understand this situation, we would like to address the following questions:

Could you please let us know how many units of the project related to this circumstance have been manufactured? How many are affected? Could you please give the failure rate? Also, could you please list the sources that you have used to design it and if it has been verified by Intel? Could you please let us know if this situation related to your design has been consulted to your BIOS developer?

In addition, could you please give pictures of the top side markings of the affected Ethernet Controllers?
