May I ask if you are using one of the devkit or you are using a custom board?
If you are using a custom board, have you change the memory size in the device tree to match with the Quartus project?

Thank you for your reponse. It's a custom board. What do you mean by 'change the memory size in the device tree', do you have a screen shot to show?
I am using an altera programmer to program a custom .bin file into a custom board using Nios2 command shell

If you are using a custom board you could not use the GSRD that is available.
You would need to change the quartus project emif parameter and the device tree below:
Line 17 to 20
