I am using an HP Envy 15t, with Windows 10 Home (64bit) and Intel HD 4600. Recently while playing games, I was receiving the error message "graphics driver stopped responding and has recovered"; after following advice found on forums to modify the tdrdelay to 8, I am now instead receiving the message "application has been blocked from accessing graphics hardware".
I see that other people have had this problem, and fixed it (?) by updating their drivers, but I am on the latest drivers (10.18.15.4248, from the HP site; seems to match with the most up-to-date on the Intel site, I also ran the software to detect any Intel hardware that needs driver updates and it didn't find anything graphics related). This seems to happen around 3-4 times a day.

The drivers on HP site for Intel HD Graphics 4600 - windows 10 seems a bit outdated.
You can try the latest driver from here: https://downloadcenter.intel.com/download/26099/Intel-Beta-Graphics-Driver-for-Windows-10-and-Windows-7-8-1-15-40-?product=80939 Download Intel® Beta Graphics Driver for Windows® 10 and Windows 7*/8.1* [15.40]
Before you install them you need to disconnect pc from internet (otherwise windows will install an older driver for your video card), uninstall the oldest drivers manually from device manager by selecting "Delete the driver software for this device", then restart windows and let windows install default windows driver.
After that restart again and install the driver, then you can connect back to internet.

Just to confirm the information above, in fact the latest driver version for Intel® HD 4600 graphics is: 4474, so, besides trying the steps above, you can also try to install the driver manually:
