Not able to apply for Devcloud extended access

I have received a mail stating that I need to fill out a survey in order to extend access to Intel Devcloud. After following the survey link, it shows following error on Apply to extend access page:

ERROR: Something seems to have gone wrong. To access the extension request form, please refer back to the original URL shared. If you continue to encounter issues, please let us know through our forum.

My devcloud username: u48215

Inspite of this, my devcloud account got expired. Can you please look into this?

We are extremely sorry. You can request for a new DevCloud account. We would no longer be monitoring this issue. Please raise a new thread if you have further issues.
