This may ondicate that a previous OpenOCD session is still running. Kill any OpenOCD console running in Eclipse (and if the problem persists any running 'openocd.exe' processes from the Windows Task Manager)
If there are no active OpenOCD sessions running try re-installing the driver using the install.bat located in C:\IntelSWTools\ISSM_2016.0.0XX\tools\debugger\driver

I had this problem, reinstalling the drivers enabled it to work.
