Issue with Starterpack survey

Hello all,

I just attempted to redeem a starterpack using a code that I received in an email from intel. I typed in the code correctly, however now I am stuck on a survey page that states "1/0 Questions". I cannot get past this screen, and when I attempted to use the code via another browser it said the key had been redeemed. Any ideas on how to proceed? Thank you!

I have followed the process and sent the details asked for to Intel support and now haven't heard from them since the 21st of August.
