Here is a project I threw together - seems to produce the right results. Self-extracting archive - 3 pcs.

Subdirectory NIOSII has the project in it to run in the Altera Cyclone 1C20 board. 

 

There is a small bit of code in there too to run in the IDE

you will need all three parts in a directory, run in a command prompt to produce the .zip file

You can use this freely and modify as you please.
