From the RAID log, drive in slot 5 failed and caused the VD offline. First of all, you should not mix SATA and SAS drives in the same enclosure. See http://www.intel.com/support/motherboards/server/sb/CS-030795.htm http://www.intel.com/support/motherboards/server/sb/CS-030795.htm. Main reason is that SAS drives have higher RPM and thus generates more vibration, which may cause SATA drives performance degrade, or even offline.

Also please make sure your http://downloadcenter.intel.com/Detail_Desc.aspx?agr=Y&DwnldID=20174 RAID firmware and http://downloadcenter.intel.com/Detail_Desc.aspx?agr=Y&DwnldID=20245 backplane firmware are updated.
