rehosting license problem "please fill in mandatory details"

I am trying to rehost my floating license for "Intel® Parallel Studio XE Composer Edition for Fortran Windows* 2020".  When filling out the Rehost/Regenerate License form, I get the following message "please fill in the mandatory details", and am unable to continue the process, even though all the information is filled in (I believe).

I see some threads on this topic, but they seem to have all been resolved through direct messages.

Please help.
