I installed Intel Media SDK on a device with HD Graphics 4000 (specifically, a Microsoft Surface Pro which only has 1 display adapter, the Intel HD Graphics 4000), but it seems there is an issue with HW acceleration support:

Tips:
- HW target does not work: If you expect it should, then make sure to install
latest Intel gfx driver, and that Intel gfx is selected as primary driver
- If Intel driver is associated with secondary adapter, make sure to
initialize DirectX device used with Media SDK with corresponding adapter

As you can see, HW acceleration is not supported though I believe my driver (9.17.10.2867) is up-to-date. I find this confusing since my devices with HD Graphics 3000 have been working well. How can I get HW acceleration to work? Thanks.

Fix: Windows Update did not get the latest Intel drivers. I went to the Intel website to get the latest drivers and now it is working.

Yes, we are aware of this issue and are working with Microsoft to ensure that future "Windows Update" drivers include full support for Media SDK API. Thank you for letting us know this affected you. Please do not hessitate to ask any questions about the MediaSDK on your Microsoft Surface Pro.
