Boot from EPCQ fails to load FPGA

I tried to boot my Nios system from EPCQ according to the following instruction: http://niosii.com/support/kdb/solutions/rd11192013_118.html

I can follow the instructions with no error, but when I restart (Power off/on) the fpga doesn't get loaded ( I included LED blinking in the sof).

Any ideas?
