Intel HD graphics driver keeps crashing my Surface Pro 4

I'm running 3 external monitors on my Surface Pro 4. At least once a day the HD graphics driver crashes and recovers.

Please make sure you have all Windows updates installed in your system, for your device, Microsoft* released a possible fix for this situation, it would appear as System Hardware Update – 12/2/2015 in the updates to be installed.
