Intel Rapid Storage RAID 5: Cannot add disk

I have a 3x1TB drive array. I have added another drive to the system and want to add it to the array. When I click on "add disk", it hangs for a few seconds then gives me an "unspecified error".

When I check the event log, I have the following message:

"IAStorUtil.StorageAction.VolumeModify action failed: FailedToClaimDisks"

Any insight as to why this is happening?
