I am sorry you are having this problems but let me help you.

Based on the report, the ig4icd32.dll may be related to openGL failure so there are some troubleshooting steps I would like you to do but I may need more information about your system configuration.

How to get it?

Dxdiag Report: Type dxdiag in Start Menu> open> Save> Attach using Advanced Editor Options.
