How to create a registration form for a program

Hello all,

I would like to know how can can I create a registration form so i can add to one of my programs. One of those things where you give your name, adress, email etc at a dialog box and when the user clicks on the button Register of the registration form and when h/she is connected to the internet all this information be sent to my email !

Any help will be greatly apreciated,

I'm sure you're able to design the form yourself. You can e-mail the info to yourself using MAPISendMail function. You can find MAPI module on my home page.
