Form Data Entry using Voice Recognition

I want to use voice recognition for filling the forms entries like Name, Age, DOB etc.

First i used detection mode but it too inaccurate. Then i made grammar using alphabets in Command and Control mode but it still not accurate (comes "B" instead of "D" due to nearly same pronunciation).

  So is there any other way to do this or i am missing something..?

Please Help.

You are experiencing a very old problem. Saying a single letter can be hard to understand. Radio people figured it all out years ago:
