I think to design a h264 core is very difficult so you cann't find a opensource of it . This is a commercial version of it http://www.wichiptech.com/h264_encoder.html

I don't believe Altera has its own version of H.264, but you can find one of its partners that offer a H.264 core: http://www.altera.com/products/ip/ampp/ampp1.html.
