How do you have these monitors physically connected to the system? What are the output port types? What adapters/dongles are you using?
Separately, please download and run the Intel System Support Utility for Windows. When you start the utility, select Everything and then click on Scan. Once it completes the scan process, click on Next and then Save. Pick a name for the text file to create and click on Save to create it. You can then close the app. Finally, open a response post here and then, using the Drag and drop here or browse files to attach dialog (below the edit box for the body of your response post), upload and attach this text file to the response post.

I managed to do the SCAN but the other concerns and i am not sure how to identify them!
Can you explain in details please
Output Types 2 VGA, 1 HDMI (Connected to a HDMI VGA converter as my desktop is old and doesn't have a HDMI port)
I attached the report and 2 pictures of the case with everything connected (Sorry not a pro)

Let me try to explain
Only any 2 of screens work together, whenever i plug the 3rd one, one of the screens goes black.
They used to work all together before but i believe an update of some kind (Driver) is causing the problem. does that make since?

Sorry, I have been busy for the past few days.
So, you have two DisplayPort ports and one VGA. Which of these ports is not working? If the problem moves around, please detail what you have tried.

when I connect any 2 screens they work, but when I connect the 3rd one, one of the screens goes black regardless of which screen or port, I think it's a driver issue no?
So when 3 screens are connected only 2 random of them work,
Each screen can work with the 2nd one but not the third

The computer is a Dell Optiplex 7010
Intel Core i5-3470 (Ivy Bridge)
HD Graphics 2500 (Driver Version 10.18.10.4425 for Windows 7 und Windows 8.1)
Windows 10 Pro 64-bit. This computer is not designed to run Windows 10, and there is no graphics driver specifically compatible with Windows 10. Have you recently upgraded to Windows 10?
Dell says:
3rd generation CPUs natively support 3 displays with the integrated CPU graphics. Three simultaneous display output requires one DisplayPort [9] with a maximum resolution of 2500 x 1600 @ 60 Hz and a DisplayPort [9] and VGA [11] port with max resolutions of 1920 x 1200 @ 60 Hz.
According to the diagnostic report, the system currently registers 2 monitors:
1920 x 1080 @ 60 Hz
1 x DVI-D
1 x VGA
Generic non-PnP monitor on HD15
1600 x 1200 @ 85 Hz
One Video Graphics Array (VGA) connector is a 3-row 15-pin HD-15 connector as shown above [11].
You have quite a few adapters in use. I understand that it worked that way before (even with Windows 10 ??), but unfortunately interoperability is more difficult to maintain every time the transmission protocol changes.
Make connections that are as straight as possible:
What ports do the other two monitors have? Manufacturer and model?
