Is it possible to upgrade from Q45/Q43 Express Chipset to a better graphics card without replacing the motherboard or the computer in general??

That depends.
Laptop? No.
Desktop? Yes, you can purchase an add-in graphics card and use it. There's a 'but', however: For systems this old, you need an old graphics card. You will need to look at what cards NVidia and AMD (ATI) made in that timeframe (say, 2008-2010) and then attempt to purchase one of these cards.

P.S. My opinion, which you can choose to ignore or not, is to upgrade the entire system. If course, this takes a significant amount of money. Consider, however, that the money you spend now on a graphics card for this old system - which would be next to useless in a modern system - will be money you won't have when it does become time to replace this old system. I call this 'throwing good money for bad'.
