IDE and breakpoint problem

I have a strange problem (not all the time, but regularly)with the Nios IDE. I am not getting notification that the breakpoint is hit and the processor stopped. What I mean is I am in debug mode and run my app. I know it should have hit my breakpoint, so I hit the PAUSE button. At that point, the IDE displays the code paused at my breakpoint. I do not understand why the IDE does not show that I hit my breakpoint and display the code window appropriately unless I hit the pause button. Any suggestions?
