1) SDR not flashed on system after integration
2) any fan failure in the system will cause all fans to run fast to compensate.
3) fan connected to the wrong fan header (which makes the systme think you have a failed fan.
Run the SELVIEW tool and check your SEL log for fan or other errors,

Fans are connected to correct fan headers, we checked that. Once the server was shutted down unexpectedly, maybe this caused this problem ?

Use selview (either the In- OS version or the EFI version) and look for critical errors in the log.
