Trying to Identify this ALTERA based motion control card manufactuer

Anybody know who made this card ISA interface 

 

I have not been able to find much of anything on my motion control card. 

 

I can tell you that it is an ISA 16 bit bus it is a full size card and it has 3 axis encoder inputs and 3 axis drive outputs (4 pin outs only 2 are used in my application) 

 

 

The controller chip is made by ALTERA it is designated as FLEX numbers on the chip are EPF8282ALC84-3 & CDA239943 

 

There is a white sticker with SE 362 B1 on it . 

 

There is a 50 pin connector sticking thru the mounting bracket, 2ea 40 pin connectors (white), 4 black connectors on the back that are not being used.
