Supply chain for silicon and dev boards

The web form on this website under contact us does not work as it says it has expired. You can be as quick as you like filling it in but it always says it has expired when you submit it.

 

So can someone please tell me where we can get the dev boards and devices from. Also datasheet and appnote information would also be useful.
