Updated ISE doc, rev 25 & PCOMMIT change blog

Revision 25 of the instruction set extension document was released, now including a change-log.

Are there any details about the new instruction set extensions added in the new document? In particular, I'm interested in AVX512_4VNNIW and AVX512_4FMAPS.
