The documentation says:
alt_avalon_sysid.h and alt_avalon_sysid_test().
But the file is named altera_avalon_sysid_qsys.h and the function is alt_avalon_sysid_qsys_test().
Also, very important, in platform designer you _MUST_ name the component "SYSID", which the documentation doesn't seem to mention.
