I had this issue with my VGA. Same error. No cures :( Anyway, check if You're NOT plugged Your JTAG adapter to USB3 port. Sometimes USB3 doesn't work.

I hit get new data and then BSOD -> Page Fault. This only happens in SignalTap all others tools (and there are some memory intensive ones I use) run fine. Didn't get this in the past on older versions of Quartus on the same machine either. I use Quartus 12.0 Build 178 05/31/2012. I cannot reproduce the error, but it seems that the bigger the SignalTap is (use of internal memory, amount of data watched), the sooner the BSOD occurs. Sometimes I can debug for several iterations (compile, debug, correct, rinse&repeat), today the Tap BSODed me on the first read after starting my PC and Quartus. Gotta ask my boss for an updated license I guess, hopefully the current build is more stable again.
