Simis,
If I'm reading that right, physical disk 0 in enclosure 0 requires reset command and the controller is predicting a failure.

What server system are you using? I would guess that it in this case you are looking for the disk in slot 1.

Hmm... just out of curiousity, which drivers are you using for the controller?
I'm no BSD guru, but I notice there's no BSD drivers on the product download page for that server board.

That driver supports LSI MegaRAID SAS 8408E and LSI MegaRAID SAS 8480E but the http://download.intel.com/support/motherboards/server/s5520ur/sb/e45274011_sr2600ur_tps_r1_9.pdf TPS (section 6.4.2) for that board quotes LSISAS1078. You might have a driver compatablity issue.

To expand on what Jason indicartes above. The difference between the LSI 8408E and 8480E controllers is just internal and external connectors. The LSI MegaRAID SAS 8408E/ 8480E controllers use the LSISAS 1068/1068E ROC.
Here's the LSI MegaRAID SAS 8408E/ 8480E controller http://www.lsi.com/downloads/Public/Obsolete/Obsolete%20Common%20Files/mr_sas_stor_ug.pdf User's Guide.

John,
I've poked around a bit on the Intel support site and I've not found any details about the SROMBSASMP2, can you point to a product landing page for that item? Or can you answer, does it use the LSISAS1078 as indicated in the TPS?

Yes, the SROMBSASMP2 or FALSASMP2 use the LSI1078 ROC. John
