It looks like this is a known problem with the evaluation version on x64 systems (see https://software.intel.com/en-us/forums/topic/531047).
I am experiencing the same error message when I type the command
ifort -v

Thanks for letting me know. As explained in the mentioned forum thread you can alternatively activate/install Intel® Parallel Studio XE 2015 products with using the license file. I'll send it to you offline. Please restart the installer and use the license file then (Activation Option: 'Alternative optiones > Use license file').

When I run parallel_studio_xe_2015_setup again, it reports
Error while extracting file C:\Program Files (x86)\Intel\Downloads\parallel_studio_xe_2015_setup\installs\composer_xe\108\w_cprovsshell_2010\VSIntShell.exe
Crc is mismatched.
Is there an uninstaller I can run first before trying setup again?

Which file is CRC mismatched? The main full offline installer package? Could you please provide the full output of the chksum of the package you downloaded?
If you kept the temporary directory when you ran the package the first time you can start the
c:\Program Files (x86)\Intel\Download\<packagid>\setup.exe
again and choose 'Remove'.

The temporary directory was deleted after the installer completed the first time.
The hash values of the setup file are

I used Windows 'Remove Program' to uninstall parallel studio. The installer then ran without error and I could use the supplied licence file.
However, I did have to make the change as described here https://software.intel.com/en-us/forums/topic/365799#comment-1757443 to get the debugger working with VS Shell 2010.
