Get error code when trying to program a Stratix 10

One of my customers gets this message when trying to program the Stratix 10 on his custom PCB:

Error(18948): Error message received from device: Detected internal error. Contact Intel Applications for further assistance. (Subcode 0x0001, Info 0x00000000, Location 0x0001AC00)

 

This only happens on one of his boards.  The SOF file is an old one that has been used on many boards prior to this.  He is using a USB Blaster to program the device.

Do you have any suggestions on what this error code means and where he should be looking to track down the problem?
