Front USB Port replacement for a SC5650BRPNA

I have a SC5650BRPNA sever chassis and the front USB ports "got" damaged and need to be replaced.

I can not find any part or part numbers so I can order a replacement.

Can someone give me the required part number or a link to a parts list that would have this information?

I have the Intel Server Chassis SC5650 Service Guide but it makes no reference to these part(s).

I think it's included in the Chassis Preventative Maintenance Kit, product code FPPTPMKIT. For more details, see http://www.intel.com/support/motherboards/server/s5520hc/sb/CS-030226.htm Spares, parts list and configuration guide.
