Cannot submit extension: 500 Internal server error

When I log into https://devcloud.intel.com/datacenter/home/ with my UUID, and click the link to extend my allocation, the form pops up and I can fill in the DevMesh project and additional information. However, once I hit submit the server always responds with a 500 when trying to load https://devcloud.intel.com/datacenter/extension_request/

Reliably happens in both chrome 80.0.3987.87 and firefox  72.0.2

My allocation expires in 4 days, so being able to submit this form ASAP is important.
