NIOS hanging when reading a register from JESD204B core

I have a customer experiencing an intermittent issue with the NIOS control of the JESD204B core. On certain modules (no apparent difference in hardware) the NIOS will hang waiting on a return from a register read inside the JESD core. The function "IORD_32DIRECT" is called, and it never returns - do you have any idea what could be causing that to happen?

Its not an expected behavior for NIOS to hang while waiting on a return from a register within our IP core. Please check if the timing constraints are specified correctly and check for timing violations. Either than timing, also check to ensure all clocks and resets related to NIOS and JESD IP are behaving correctly.
